Child Release of Liability Waiver
PARTICIPANT HOLD HARMLESS: I (we) hereby release, hold harmless, and indemnify For The Culture Sports, Inc., along with its directors, officials, agents, employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of the premises used to conduct the event, from any and all liability, damages, costs, expenses, claims, and settlements, including, but not limited to, attorney’s fees arising out of the use of the facilities during my (our) participation in the programs or events of For The Culture Sports, Inc., or in connection with or growing out of the treatment or care from nurses, doctors, hospitals, or other medical units for any and all injuries I (we) sustain while using any equipment of or participating in any activity sponsored by For The Culture Sports, Inc.
ASSUMPTION OF RISK: I (we) understand and acknowledge that participation in For The Culture Sports, Inc. programs may involve serious risks, including, but not limited to, bodily injury, permanent disability, paralysis, and death. I (we) voluntarily assume all such risks, whether known or unknown, arising from participation in these programs or events, and assume full responsibility for my (our) participation.
MEDICAL AUTHORIZATION AND RELEASE: In the event of an emergency or injury, I (we) authorize For The Culture Sports, Inc. representatives to take all necessary actions, including securing emergency medical treatment, which may include but is not limited to emergency services at the nearest hospital. I (we) understand that my personal insurance will be used in such cases, and For The Culture Sports, Inc. is not responsible for any medical expenses incurred.
MEDIA RELEASE: I (we) understand that For The Culture Sports, Inc. may use photographs, digital images, and/or videotape of participants and spectators to publicize and promote recreational opportunities for For The Culture Sports, Inc. I (we) grant permission for such use without compensation and acknowledge that all images are the property of For The Culture Sports, Inc.
PARENT/GUARDIAN CONSENT: As the adult registrant or the parent/legal guardian of the participant(s) named below, I (we) grant permission for myself and/or my child(ren) to participate in For The Culture Sports, Inc. programs. I certify that I (we) am/are in good health and capable of participating in all activities.
SEVERABILITY: If any provision of this waiver is found to be invalid or unenforceable by a court of competent jurisdiction, the remainder of this waiver shall remain in full force and effect.
GOVERNING LAW: This waiver is governed by the laws of the state in which For The Culture Sports, Inc. is located, and any legal actions arising out of participation shall be filed in the courts of that state.
By signing below, I (we) acknowledge that I (we) have read and understood this Release of Liability Waiver and agree to its terms.
